Who can make a referral?

Partner agencies, VA and public-service staff, community organizations, and case managers. Veterans and family members can also send an inquiry directly.

Does submitting a referral guarantee placement?

No. Capacity is limited and every submission is reviewed individually. Whatever the outcome, we aim to communicate it clearly.

Which counties do you serve?

We are primarily serving Rockdale, Newton, DeKalb, and Gwinnett counties in Georgia.

Do you serve areas outside these counties?

We are primarily serving the four counties above and welcome inquiries from surrounding areas. Please contact us to ask about your area. An inquiry does not guarantee coverage or placement.

What information do I need to share?

Only what is needed to begin a conversation. Please do not include Social Security numbers, DD-214s, or medical records in any form.
